One comment on DIA report. I served in military intelligence for 18 of 22 years so take this information for what it is. Since this target was likely approved by the POTUS, DIA would take the lead for Battle Damage Assessment (BDA). Typically the Joint Force Commander would be responsible and the COCOM would provide BDA for theater targeting.
Bottom line DIA is responsible for performing BDA for military targeting. They know the munitions and their effects. The questions that must be asked post strike: A recommendation for another strike based on BDA? If there was, why was it not authorized? If the recommendation was not provided, why not? To that end, what was the purpose of the strike?
